Use this form if you have CardExchange that has been working and under the following circumstances requires relicensing.
NB: If you have tried to activate CardExchagne for the first time and received the error "Cannot connect to the license server (error code 10019)." Please go to the Online Activation Conversion Form.
a) The PC on which CardExchange is faulty and you need to move the software to a new PC
b) You need to move the software to a new PC and will delete the old installation
c) Your CardExchange license file is corrupt and you are using CardExchange 2.x and you have not backed up your license file.
d) You have had to re-install the operating system and you are using CardExchange 2.x
e) You have had to re-install the operating system or your CE file is corrupt and you are using CardExchange 3.x and have not backed up your license file.
f) You have been instructed to relicense your CardExchange Software by a PPC technician.
